The inability to determine actual costs for care and exorbitant fees for simple services are two of the most frustrating issues patients (including us) encounter in medicine today. We promise that these will not be concerns for you, with respect to the services you receive in our office, as a member of Uncommon Healthcare.
Upon enrollment, payment for two full months of membership will be collected in addition to a prorated fee for the month during which you are enrolling. Thereafter you may elect to be billed monthly, quarterly, biannually, or annually.
Children* 0-19 years of age $50 per month
*with at least one parent member
Adults 20-44 years of age $80 per month
Adults 45-54 years of age $100 per month
Adults 55-64 years of age $120 per month
Adults 65 and over $150 per month
House calls $50
Special pricing is available for patients paying annually and corporate members (business groups of 5 adult members or more). Please contact us for details.
